Can reducing loneliness prevent or reverse frailty?
The corpus does not support a settled conclusion that reducing loneliness prevents or reverses frailty. While longitudinal evidence suggests a bidirectional relationship where baseline social vulnerability predicts faster frailty progression, the evidence for prevention is unresolved (Hanlon et al., 2024). A large longitudinal study of Dutch older adults found that earlier frailty consistently predicted subsequent loneliness, but the reverse—loneliness predicting future frailty—occurred only at specific intervals (Mehrabi et al., 2024). Similarly, a study of Chinese older adults found no significant difference in frailty progression for those with transient or chronic loneliness compared to those without loneliness (Chu & Zhang, 2022). However, participants with chronic loneliness were less likely to experience a reversion in their frailty status, with an odds ratio of 0.68 (Chu & Zhang, 2022). The corpus contains one small prospective intervention reporting reduced frailty scores over 12 months following a loneliness and falls intervention. One non-randomised study cannot support a preventive claim, and the better-supported path runs from physical improvement to reduced loneliness. A randomised trial with frailty transition as the primary outcome and loneliness as the target would be required to answer this question. (Research Assistant Note: Frailty itself may be modifiable through combined exercise, nutrition, and cognitive interventions, but this is distinct from a loneliness-led intervention).
